Please fill-in the requested information using Acrobat Reader, and then print it out and have your sponsor, advisor, or supervisor sign it.
Engineering Students should drop off the completed form at the Engineering Science general office in ASB 9801 to be reviewed and authorized by EnSc staff. All others should put completed and signed applications at the office of the Dean of Applied Sciences (ASB 9861). Please mail gripe@fas.sfu.ca with account inquiries.
